  • Online: Sign in or create an account at TennCare Connect.
  • By Phone: Call TennCare Connect at 855-259-0701.
  • By Mail/Paper: Download a paper application from the TennCare How to Apply Page and mail or fax it in.
  • In-Person: Visit a local Department of Human Services (DHS) office.
