You can find the payer claim control number (also called an Internal Control Number or ICN) on the Electronic Remittance Advice (ERA / 835 file), the paper Explanation of Benefits (EOB), or the insurance payer’s online provider portal.

Where to Look

  • Electronic Remittance Advice (ERA / 835): Found in the CLP02 segment or the REF segment with the "1K" qualifier for payer claim control.
  • Paper Remittance Advice or EOB: Printed next to or near the patient's name and member/beneficiary ID.
  • Payer Online Portal: Listed directly under the claim details or claim status history page after the payer processes the submission.
  • Billing Software: Automatically imported and stored within the specific claim's transaction or payment details view if you accept electronic ERAs.
