How do I submit documents to Medicare?

Editorial team Published on

You can submit documents and forms online through the Social Security Administration Portal if you are applying for Medicare or submitting enrollment forms like Part B.

Because Medicare enrollment and claims management are split between different agencies, the way you upload or submit documents depends on what you are trying to send:

For Medicare Enrollment & Social Security Forms (Part B, Employment Proof)

  1. Go to the Social Security Administration and log into your My Social Security account.
  2. Navigate to the Medicare tab or the "Manage benefits" section.
  3. Select the option to sign up for benefits or upload documents (such as Form CMS-40B or CMS-L564).
  4. Drag and drop your saved PDF files or click Choose Files to attach your documents.
  5. Review your attachments and click Submit, then save your confirmation page.
  6. Alternatively, you can use the direct SSA eSubmit Portal for certain requests.

For Marketplace / Affordable Care Act Documents

  • If you received a notice from the Health Insurance Marketplace requiring verification documents for your application, log into your account at HealthCare.gov to upload files directly next to your data-matching issues.

For Original Medicare Claims and Medical Records

  • Original Medicare claims are typically filed by your doctor, provider, or supplier rather than uploaded directly by you.
  • If you need to manually file a claim or submit medical records yourself, Original Medicare usually requires you to mail or fax a Patient Request for Medical Payment (CMS-1490S) form along with itemized bills to your state’s Medicare Administrative Contractor (MAC) rather than uploading them online.
