To fill out a standard medical insurance claim form (such as the universal CMS 1500 Form), you must complete three core sections covering patient demographics, medical diagnosis and procedure codes, and provider details.

Step 1: Patient and Insured Information (Top Section)

  • Box 1 & 1a: Check the appropriate type of health insurance coverage (e.g., Medicare, Medicaid, Group Health Plan) and enter the insured's unique member ID number from their insurance card.
  • Boxes 2, 3, & 5: Enter the patient's full legal name (Last, First, Middle Initial), date of birth (MM/DD/YYYY format), sex, and complete mailing address.
  • Boxes 4, 6, & 7: Indicate the policyholder's ("insured") name and their relationship to the patient (mark "Self" if the patient owns the policy, or "Spouse"/"Child" if dependent) along with their address.
  • Boxes 12 & 13: Provide the patient and insured signatures or write "Signature on File" (SOF) to authorize the release of medical information and direct payment to the provider.

Step 2: Diagnosis and Treatment Details (Middle Section)

  • Box 14 & 15: Enter the date of the current illness, injury, or first symptom onset.
  • Box 21: Enter your standard ICD-10-CM diagnosis codes describing the patient's condition or nature of illness.
  • Box 24 (A–J): Itemize the specific services rendered on each date of service using standard CPT or HCPCS procedure codes, listing the place of service, charges, and the rendering provider's National Provider Identifier (NPI) number.

Step 3: Provider Billing Information (Bottom Section)

  • Box 25: Provide the billing provider's federal Tax ID number (EIN or SSN).
  • Box 28–30: Sum up the total charges, subtract any prior payments, and list the final balance due.
  • Box 31 & 33: Include the provider's signature/credentials, date, and the complete billing name, address, phone number, and NPI of the practice or facility.
