How to fill the insurance claim form?
You can find official instructions and standard layout guidelines through the National Uniform Claim Committee if you are using the standard medical CMS 1500 Claim Form Instructions Tool.
To fill out a standard health insurance claim form (such as the CMS-1500), break the process down into three main sections: patient demographics, treatment details, and provider information.

- Box 2 & 3: Enter the patient's full legal name (Last, First, Middle Initial), date of birth, and sex.
- Box 4, 5, 6, & 7: Provide the primary insured's name, the patient's address and phone number, and the patient's relationship to the insured (self, spouse, child).
- Box 12 & 13: Sign or write "Signature on File" (SOF) to authorize the release of medical information and assign medical benefits.
2. Treatment and Medical Details (Middle Section)
- Box 21: Enter the official diagnostic codes using the correct indicator (such as ICD-10).
- Box 24A-24J: Provide line-by-line service details:
- Dates of service
