You can find or retrieve your login details on the CAQH Provider Data Portal.

How to Retrieve Your Username

  • Go to the login page on the CAQH Provider Data Portal.
  • Select "Forgot Username".
  • Enter your unique CAQH Provider ID number (assigned during registration).
  • Agree to the terms of service and complete the verification prompt.
  • View your username, Provider ID, and primary email on the screen.

Important Distinction

  • Your username is the custom name you created to sign in.
  • Your CAQH Provider ID (or CAQH Number) is a permanent, career-long identification number, but it is not the same as your login username.
